  • Student Information & Emergency Contact Forms

  • Birth Certificate or Proof of Age

  • Immunization and Health Records
    In accordance with state guidelines.

  • Academic Records
    Most recent report cards or transcripts from previous schools.

📚 Academic Placement

  • Students may be required to complete a placement or entrance assessment.

  • Academic records are reviewed to ensure appropriate grade-level placement.

  • Placement decisions are made in the best interest of the student’s success.

✝️ Faith & Values Commitment

  • Families must acknowledge and support Academere’s Christ-centered mission and values.

  • A Statement of Faith or Values Agreement may be required.

🧑‍🏫 Interviews & Meetings

  • A family interview or student meeting may be scheduled as part of the admissions process.

  • This helps ensure a strong partnership between the school and family.

🎓 Transfer Students

In addition to general requirements, transfer students may be asked to provide:

  • Transfer or withdrawal form from the previous school

  • Additional academic or discipline records

  • Mid-year transfer approval (based on availability)
