    Includes emergency contacts and family details.

  • Parent/Guardian Agreement
    Acknowledgment of Academere’s policies, mission, and Christ-centered values.

  • Code of Conduct Agreement
    Commitment to uphold school expectations for behavior, discipline, and respect.

  • Medical & Emergency Information Form
    Health history, allergies, medications, and emergency authorization.


📚 Academic Records

  • Previous School Report Cards or Transcripts
    Most recent academic records for placement and evaluation.

  • Teacher Recommendation Form (if applicable)
    Provides insight into the student’s learning style and behavior.

  • Standardized Test Scores (if available)
